Our services may include: visits for routine primary care, acute illness, post-hospitalization/post-skilled nursing facility care, and disease management; care plan oversight; dementia assessments; medication management and review; and referrals for specialists, blood work, and x-rays.

We treat and help manage a wide range of chronic, complex conditions, including:
- Dementia, including Alzheimer’s disease
- Chronic obstructive pulmonary disease (COPD)
- Diabetes
- Heart disease, including heart failure and coronary artery disease
- Movement disorders such as Parkinson’s disease and ALS
- Recovery after stroke
- Bedsores and chronic wounds
Our team includes geriatricians, nurse practitioners, nurses, and other health professionals who have training and experience in geriatric medicine. We personalize your care based on your goals and preferences.
We deliver onsite care to residents in assisted and independent living facilities, coordinating with your facility staff to keep you healthy.
We help connect you with the services you may need, such as x-rays, EKGs, or referrals to other specialists through our established relationships within the community. Our responsive, coordinated care model with our partners helps prevent hospitalizations.
Whenever possible, the same providers come for your regular visits. We build a close relationship with you and your family so that we are better able to understand your needs and provide personalized care.
Onsite primary care visits can be scheduled Monday-Friday during regular business hours.
