At Transitions LifeCare, we believe that every individual deserves the quality of life he or she wants when facing a serious illness or life’s end. We serve every eligible patient, regardless of their ability to pay. With a gift to Transitions LifeCare, you turn your care into comfort, your desire to help into hope, and your donation into dignity for a friend or loved one – and their family – at the end of life. We help patients live as fully and comfortably as possible and maintain a sense of dignity throughout their lives.
We are a qualified 501(c)(3) charitable organization; donations are fully tax-deductible as allowable by law. Our Tax ID number is 56-1228779.

Gifts Through Your Will or Estate
Planned giving, also known as gift planning or deferred giving, refers to charitable gifts that require some advanced planning before they are made. Some examples include philanthropic gifts through your will (a bequest), retirement assets, or life insurance. Others include gifts of appreciated stocks and securities, real estate and personal property, trusts, and pooled income funds.
A planned gift is a very simple way to support Transitions LifeCare. A variety of giving strategies can be leveraged to accommodate your specific needs and circumstances while building a personal legacy that will benefit our patients and families now and for generations to come.
Though we cannot provide legal and tax advice, we are available to work with you and your financial and legal advisors to establish planned gift arrangements that accomplish your personal and charitable objectives and maximize your legacy.

Designated & Special Gifts
Healing Spaces
Transitions LifeCare offers the only inpatient hospice facility in Wake County. Built in 2010, the William M. Dunlap Center for Caring (Hospice Home) is due for upgrades to key systems and enhancements that will bring greater patient and family comfort. The Healing Spaces Renewal Project will refresh and renew the environment at the heart of our mission. Project priorities include replacing aging HVAC systems and replacing hospital beds in the Hospice Home, refreshing our Administration Building’s meeting and event spaces, creating an accessible playground for kids, and installing additional campus security systems.
Community Indigent Care Program
This program allows Transitions LifeCare to extend all of our services to patients not eligible for Medicare/Medicaid, not covered by private insurance, and without other means of paying for services. This program also helps patients and families with emergency needs, such as assistance with paying rent or a utility bill.
Grief Center
This program supports Transitions GriefCare, which offers individual counseling and support groups for those who have experienced the death of a loved one and helps us serve grieving families, adults, and children. Grief counseling is offered at no cost to families served by Transitions LifeCare as well as any member of the community.
Hospice Home Patient Fund
We provide inpatient hospice beds at our William M. Center for Caring at reduced daily rate for patients who do not qualify for intensive inpatient care, covering room, board, and 24/7 skilled nursing during their final weeks of life. The true cost of this care is often more than twice this rate, and philanthropic gifts help bridge the gap. Your gifts to this program directly relieve families of a heavy financial burden, ensuring they can focus on their loved one. No one should worry about the cost of care at the end of life, and we are committed to reducing that burden for every patient we serve.
Endowment Fund
The Endowment Fund provides an important measure of financial stability for Transitions LifeCare by creating a predictable and steady source of income for generations to come. This income stream can be used to underwrite the cost of current programs, as well as respond to future needs arising from growth in demand for hospice care and expansion of our service area.
Transitions Kids
Transitions Kids offers various levels of care designed solely around the unique needs of children facing life-limiting illness. The goal is to provide home-based supportive care services for the child and family that are focused on enhancing quality of life by reducing physical and emotional pain and distress.
