Transitions LifeCare Featured on “Community Matters”

Transitions LifeCare recently had the pleasure of being featured on CW22’s Community Matters to talk about the services we provide and the many resources available to our community.

Chief Medical Officer Dr. Rachel Lipner shared our organization’s origin story and reflected on our deep roots in the community after more than 40 years of service. She discussed what makes us unique as a nonprofit, how our team helps patients and their families navigate the complexities of the healthcare system, and gave insight into our palliative care program—explaining the qualifications and how it differs from our other services.

 

 

Meredith Jones, director of family support and clinical services, spoke about how we personalize the care and support we provide depending on each patient and family’s unique situation. She emphasized the range of services we offer beyond medical care, highlighting the comprehensive circle of physical, emotional, and spiritual support available to those facing serious illnesses.

 

 

Dwan Kelsey, associate director of community engagement, discussed the importance of engaging with different community groups and developing relationships to better understand how we can meet their needs. She also shared how employees work as a team to provide each patient and family with a well-rounded circle of care.
