The Yeargan Foundation Announces $1,000,000 Gift and Matching Challenge

The Yeargan Foundation is making a $1 million gift to Transitions LifeCare. This generous gift will directly support medical care, emotional support, and bereavement services for families across our community.

The Foundation is also challenging the community to match this $1 million gift by December 31, 2025. “We will match a gift of any size that’s designated to Transitions Kids, or any unrestricted gift of $1,000 or more. But, even if your gift doesn’t meet the matching criteria, we urge you to send your most generous gift today,” said Polly Yeargan Hardie. Every single dollar counts to ensure that Transitions LifeCare can continue to provide compassionate care for anyone facing serious illness or the end of life—regardless of their ability to pay.”

“Like many people, I first became aware of Transitions LifeCare after they provided compassionate care to a relative of mine at the end of life. Our family was so impressed by the services we received that we wanted to learn more about the organization,” said Polly Yeargan Hardie.

“In late 2014, I visited the William M. Dunlap Center for Caring, the agency’s inpatient hospice home, and talked with their leadership team. I wondered aloud if they had ever considered a specific program for children facing terminal illness. I was told they hoped to one day provide services to the pediatric population and already had many inquiries from local hospitals. I was very interested in fast-tracking the launch of this program and was pleased that we were able to make a foundational gift to formally launch Transitions Kids in September of 2015.”
