Transitions LifeCare Earns Medicare’s Highest Five-Star Quality Rating for Hospice Care
RALEIGH, N.C. (August 26, 2026) Transitions LifeCare is proud to announce that it has received Medicare’s highest possible Five-Star Quality Rating for hospice care, a distinction that reflects the organization’s unwavering commitment to providing compassionate, high-quality care for patients and families throughout their healthcare journey.
The Medicare Five-Star Quality Rating is based on feedback from families and caregivers about their experience with hospice care, recognizing organizations that consistently deliver exceptional service, communication, and support during one of life’s most meaningful and challenging times.
