Transitions LifeCare receives a $75,000 grant from The Leon Levine Foundation
The Leon Levine Foundation awarded Transitions LifeCare a $75,000 grant to support their Transitions Kids program which provides pediatric hospice, palliative care, and bereavement services to children and their families in the greater Triangle area.
Transitions LifeCare, a nonprofit organization serving Wake and seven surrounding counties, was founded in 1979 as Hospice of Wake County. For more than 46 years, the organization has remained committed to providing compassionate care to everyone in need, regardless of financial circumstances. “We’re grateful to The Leon Levine Foundation for its continued support through this generous grant award,” said Transitions LifeCare’s CEO Dr. Adam Wolk. “This year marks the 10th anniversary of the Transitions Kids program. Established in 2015 to address the lack of community-based hospice, palliative care, and bereavement services for children and teens in the Triangle, the program is designed to meet the unique needs of children and families facing serious illnesses and loss.”
Today, Transitions Kids remains the only community-based program in central North Carolina that offers hospice, palliative care, and bereavement support to children.
“Our goal is to enhance the quality of life for children with a life-limiting illness by reducing their pain and suffering,” says Wolk. “These funds allow us to fulfill our mission of caring for children and their families, regardless of their financial status or ability to pay.”
Wolk emphasizes the importance of community involvement: “We could not provide our urgently needed services without the trust and investment of our community. We are grateful to The Leon Levine Foundation for its continued support and belief that the care we provide is essential to the well-being of our community.”
