Transitions LifeCare Receives $2.5 Million Gift to Support Transitions Kids Program

transitions kids logoRALEIGH, N.C. — Transitions LifeCare is honored to announce a generous $2.5 million gift from a family who wishes to remain anonymous. This heartfelt contribution, pledged over the next five years, comes from a family profoundly impacted by the care they received and is designated to support Transitions Kids—our program providing hospice and palliative care for children.

“This gift is incredibly moving,” said Dr. Adam Wolk, CEO of Transitions LifeCare. “It reflects not only the power of compassionate care but also the lasting bond that forms between our team and the families we walk alongside. We’re so grateful for this family’s trust and belief in our mission.”

Transitions Kids provides specialized, family-centered care for children with life-limiting illnesses, focusing on comfort, dignity, and emotional support. Offered at no cost to families, the program is one of the few in the region dedicated solely to pediatric palliative and hospice care.

Unfortunately, pediatric palliative and hospice insurance reimbursements often fall short, leaving families and care providers to navigate complex challenges with limited resources. As a result, organizations like Transitions LifeCare rely heavily on philanthropy to ensure these essential services remain available to all who need them.

In 2024, Transitions LifeCare provided $2.6 million in uncompensated care—ensuring that no family was turned away, regardless of financial situation. This gift will help sustain and grow Transitions Kids, allowing the organization to serve even more families across central North Carolina.
