Transitions LifeCare Surpasses $1 Million Yeargan Foundation Challenge to Support Transitions Kids Program

Transitions LifeCare announced today that it has exceeded The Yeargan Foundation’s $1 million challenge gift, raising a total of $1,195,000 from more than 330 generous donors in support of its Transitions Kids program.
Established ten years ago, Transitions Kids provides compassionate, specialized care to children and families facing serious illness. The matching challenge from The Yeargan Foundation—created in honor of the resilience of Transitions Kids patients and the dedication of the care teams—has helped accelerate the program’s growth and expand its reach in the community.
“We are thrilled with the results of The Yeargan Foundation challenge and are so grateful to the community for its unwavering support of Transitions LifeCare,” said Polly Hardie, Trustee of The Yeargan Foundation. “We made this matching gift in honor of the strength and resilience of the Transitions Kids patients and their families, and in gratitude for the care teams who surround them with compassion. It is my hope that this support helps expand the reach of a program that brings care, comfort, and peace when it is needed most.”
Funds raised through the challenge will directly support:
- Pediatric medical care for children with complex or life-limiting conditions.
- Emotional and spiritual counseling for families navigating serious illness.
- Bereavement services to help parents, siblings, and caregivers heal.
This milestone reinforces a legacy of care that began with the organization’s founding in 1979. For more than four decades, Transitions LifeCare has provided compassionate, comprehensive care to individuals and families, regardless of their ability to pay.
