To those of you that I don’t know, let me quickly introduce myself. My name is Lani Tillar-Smith, and I am a Senior Provider Relations Representative with Transitions LifeCare. I’ve been with TL since 2010, and I am truly committed to our mission of providing excellent care. My personal mission is to always provide my referral partners with excellent service, and I am so thrilled to be your main, non-clinical first point-of-contact for Transitions LifeCare. This page was created as a resource for you: a place where you can get updates, find resources, and communicate directly with me. I hope having a central place to go for quick, streamlined communication and resources will make it easy for you to get the information you need quickly and easily.
